You must prove you have other health insurance coverage so you don’t get a late enrollment penalty!
3 Months Before
Confirm you have Medicare Part A on your Medicare red, white & blue card
You need TWO forms for your late enrollment in Part B.
Ways to apply:
- Online: You will need proof that you had/have credible health coverage. Go to online application.
- Paper: The 2 forms you need are – CMS-40B Get that here and CMS L564 Get that here.
- Mail, Fax or Drop Off in-person to your Local Social Security office
Decide on your secondary insurance | the 20% Medicare doesn’t cover
You can choose an Advantage or a Supplement plan.
If you CHOOSE a Supplement(Medi Gap) Plan
Supplements are taken at any Medicare participating provider. There is NO NETWORK!
You will need a separate Part D plan – make sure your current medications are on its formulary or you will pay retail price for them.
If you CHOOSE an Advantage Plan
Make sure your Primary & Specialists are in the Network
Most Part C plans include a drug plan. Make sure your medications are on the formulary.
